Please complete as much of the following form as possible. Often the completeness of the request determines the speed and accuracy of acquisition. "Submitting" the request mails the form to the Collections, Reference, Instruction and Outreach office, 106 Olin Library, and requests will be forwarded to the appropriate selector. You may also correspond directly with library subject selectors.

If the item is to be placed on reserve for class use you do not need to fill out this form. When you fill out the course reserve form you may indicate that you wish to have the library order the item.
